*PURPOSE: The goal of this service line pilot was to extend clinical transplant pharmacist specialty services to solid organ transplant (SOT) patients cared for by non-SOT hospital teams across a large health-system. *METHODS: An electronic list of SOT patients admitted to non-SOT hospital teams across the Midwest region of a health system was created and generated daily during the three-week pilot period. One transplant pharmacist provided clinical review daily of all patients and consultation to the non-SOT primary team as indicated. After the pilot, a satisfaction survey was sent to members of the non-SOT primary teams. *RESULTS: Data presented in Table 1. During the three-week pilot, 53 SOT patients were reviewed generating 80 clinical recommendations across 33 unique patients (62%). Of the 80 recommendations, 46 (57.5%) involved therapeutic drug monitoring, 9 (11%) related to drug interactions, and 5 (6%) related to immunosuppression formulation selection. The remaining 18 interventions spanned unique, clinically impactful, transplant-related medication issues depicted in Figure 1. There were 9 responses to the satisfaction survey. Interaction with the transplant pharmacist changed plan of care for 67% of respondents and positively impacted patient care for 100% of respondents. All respondents indicated that permanent implementation of the service would improve patient care. *CONCLUSIONS: Adding transplant pharmacist specialist services to the care of SOT recipients admitted to non-SOT hospital services positively impacted patient care through clinically relevant, transplant-related interventions. Care was cost-efficient with the entire health-system Midwest region cared for by one specialized pharmacist. Most commonly, the transplant pharmacist improved therapeutic drug monitoring and provided expert assessment of drug interactions for high-risk transplant medications with which non-SOT hospital teams are often unfamiliar. Transitions of care were also improved through increased execution of transplant monitoring and referral to appropriate transplant center providers for post-dismissal follow-up when indicated. Future consideration for the implementation of a transplant pharmacist in SOT patient care across medical settings warrants exploration. [
