*PURPOSE: Good outcomes have been demonstrated in deceased donor kidneys coming from donors with acute kidney injury (AKI), including those requiring temporary dialysis at the time of donation. There is however, limited data on the impact of donor dialysis duration and kidney transplant outcomes. *METHODS: We examined outcomes in AKI kidneys from dialysis donors transplanted at our center between 2010-2019. All AKI kidneys underwent a preimplantation biopsy review. Outcomes were compared between donors on dialysis 1-2 days, 3-4 days and >5 days. *RESULTS: During this time, 155 kidneys were transplanted from donors on dialysis (1-2 days, n=88; 3-4 days, n=47; >5 days, n=20). The median donor dialysis duration was 2 days (IQR 1, 4), with 13 days being the longest observed duration. There were no differences in donor age (p=0.84), KDPI (p=0.89), cause of death (p=0.21) or mechanism of death (p=0.25) between the 3 groups. There were also no differences in time-zero biopsy findings (ci, p=0.06; ct, p=0.28; cv, p=0.23, ah, p=0.51). CIT were similar between the 3 groups (p=0.29). DGF was observed in most recipients (96.6% vs. 85.1% vs. 85.0%, p=0.04). There were no differences in duration of DGF between the 3 groups (Figure 1, p=0.63). There were also no differences in hospital LOS (p=0.14) and early rejection events (p=0.27). At 1-year, there were no differences in eGFR (p=0.97), patient (p=0.46) or graft (p=0.22) survival (Figure 2). One-year death-censored graft survival was 95.5%, 100%, and 85% for donor dialysis duration of 1-2 days, 3-4 days and >5 days (p=0.28). *CONCLUSIONS: Outcomes of transplanting AKI donor kidneys are similar between donors on shorter and longer periods (up to 13 days) of temporary dialysis prior to donation. The excellent outcomes observed are likely reflective of careful preimplantation biopsy review and selection. [
